Application for Integrated Forest Management Agreement

 

Requirements

Individual

  1. Duly accomplished Application Form
  2. Application Fee
  3. Proof of Financial and Technical Capability to undertake initial activities in forest plantation establishment and development, in the form of credit lines from financial institutions, collateral properties; or good past performance/track records in the development and management of tenurial instruments
  4. Appropriate clearance from the National Commission on Indigenous Peoples (NCIP)

Corporation

  1. Duly accomplished Application Form
  2. Application Fee
  3. Certified copy of Securities & Exchange Commission (SEC) Registration and/or Cooperative Development Authority (CDA)
  4. Articles of Incorporation and By-Laws, including the list of present offices and stockholders duly certified by the Board Secretary
  5. Audited Financial Statement for the last (2) preceding years
  6. Resolution of the corporate governing body (Board of Directors, Board of Trustees, etc.) designating the authorized representative of said corporation, association or partnership to apply/sign documents for and in behalf of the company
  7. Proof of Financial and Technical Capability to undertake initial activities in forest plantation establishment and development, in the form of credit lines from financial institutions, collateral properties; or good past performance/track records in the development and management of tenurial instruments
  8. Appropriate clearance from the National Commission on Indigenous Peoples (NCIP)
